What Nobody Told Me About Manual Wheelchairs: A Guide to Power Add-Ons That Can Change Everything

I spent six months borrowing a wheelchair that didn't fit my body. I went through insurance appeals, lost faxes, and paperwork I had to track down myself while managing hEDS, POTS, and everything else that comes with daily life in a body like mine. By the time my own chair finally arrived, I had learned more about the durable medical equipment process than I ever wanted to know.

But there's one thing I learned after I got my chair. One thing that didn't come up in the mobility evaluation, wasn't in any of the paperwork, and wasn't something anyone handed me a brochure about.

My manual wheelchair can have a motor.

Not a power wheelchair. My chair. The one I already have. There are devices that attach to it and provide powered assistance, and I had no idea they existed until I went looking on my own. I spent hours watching videos online, searching for someone who looked like me and lived like me and could show me what this life could actually look like. Content that matched my reality was hard to find.

So I'm writing the post I wish I had found.

This is not a sponsored post. I don't have personal experience using every device on this list. What I do have is a lot of time researching, a design background that makes me pay close attention to how things fit and function in real spaces, and a body that gives me a specific perspective on why these things matter. If you're living with hEDS, POTS, chronic fatigue, hypermobility, or anything else that makes manual propulsion complicated, I hope this helps.

First: What Are Power Add-Ons?

Power add-ons are devices that attach to your existing manual wheelchair and provide motorized assistance. You keep your chair. You keep the portability, the lightness, the familiarity of what you already have. You just get support when you need it.

These are not the same as a power wheelchair. For many of us with dynamic disability, fluctuating energy, or conditions that affect upper limb stability, a full power wheelchair isn't always the right answer. It can solve one problem and create three others, whether that's fitting in your car, navigating your home, or simply not feeling like you've lost the part of your mobility that still works.

Power add-ons exist in the space between "push yourself everywhere" and "switch to a power chair." That space is bigger than most people know.

There are three main categories: rear-mounted assist, power assist wheels, and front attachments. I'll cover all three.

Category One: Rear-Mounted Power Assist

These devices attach to the back of your chair and push you from behind. You still steer by propelling your wheels normally. They are the most common type covered by insurance and generally the most discreet option.

A quick note on insurance: these typically fall under HCPCS code E0986. Medicare and many private insurers cover them via capped rental over 13 months rather than a lump sum. Coverage isn't guaranteed, and the process looks a lot like what I described in my wheelchair series. An ATP (assistive technology professional) and a letter of medical necessity are generally required. Ask your insurance directly before assuming anything.

Permobil SmartDrive MX2+

The SmartDrive is probably the most widely known rear-mounted option in the US. It's a compact unit that clips onto the back of your chair and is controlled by a set of wired buttons you tap with your hand on either side to start it, steer normally, and tap to stop. The PushTracker wristband has been phased out, so the wired SwitchControl is currently the primary way to operate it.

It connects to Apple Watch and Android smartwatch apps with gesture controls and has activity tracking built in. There was also a SpeedControl Dial option, but that has been recalled and is no longer available. And if you have a newer Android watch, check compatibility before you commit. My Samsung Galaxy Watch 7 was too new to work with their app. It's a frustrating gap for a device at this price point, and I'm hoping Permobil updates their app to catch up with current hardware.

It's the lightest rear-mounted power assist attachment currently on the market, works with most rigid and folding chairs, and has strong US-based support through Permobil.

The trade-off: it does add length behind your chair. In tight spaces, that matters.

Good for people who want the lightest option available with a long track record in the US market, and who have confirmed their phone or watch is compatible before purchasing.

Alber SMOOV One

The SMOOV One is the SmartDrive's closest competitor. It's a rear-mounted drive unit controlled by a wireless dial that mounts to your frame. You turn it to accelerate, release to slow down. It connects to an iOS and Android app via Bluetooth for settings, activity tracking, and unlocking higher speeds.

Reviewers consistently describe the ride as smoother than the SmartDrive, and it has a higher top speed at 6 mph with a range of about 12 miles. But there's something worth knowing before you commit: this is a German product, and US-based repair and parts support has been inconsistent. Some users have reported waiting months for replacement control units. If this is your primary mobility tool, a long repair wait is not a small thing.

Good for people who want a smooth, intuitive ride and either have access to a dealer with strong Alber support or are comfortable with the risk.

Empulse R90 by Sunrise Medical

This is the one I wish someone had told me about from the beginning.

The R90 attaches at the rear of the chair, but it sits underneath the seat rather than sticking out behind it. The motor sits centrally between your rear wheels. Your chair's footprint stays exactly the same. Nothing protrudes beyond your wheels.

That might sound like a small thing until you think about tight bathrooms, narrow doorways, restaurant aisles, getting your chair into a car, or transferring. In all of those situations, a device that doesn't add to the length of your chair is meaningfully different.

It's Bluetooth controlled. No wires running along your frame. The control box is simple push-button and mounts wherever is easiest to reach. The sensor technology reads your push force and adjusts its assistance accordingly, so it responds to your effort instead of running at a fixed output.

One of its most practical features is the lift-up wheel. When you don't need assist, you press a button and the drive wheel raises so there's zero drag while you propel manually. This means you can use it outside, lift it when you go into a store, and lower it again when you head back out, all without removing the device. For people who only need assist in certain situations, that flexibility matters.

The removable battery can be upgraded to an extended version with a range of up to 19.5 miles. It weighs about 11 pounds. It's available through Sunrise Medical dealers in the US and comes with a 24-month warranty.

Good for people who want power assist without changing their chair's dimensions, who value a clean wireless setup, and who want to move between powered and manual easily in the same outing.

Category Two: Power Assist Wheels

These replace your existing rear wheels with motorized versions. The motor lives inside the wheel hub and amplifies your push in real time. The more force you put in, the more power you get out. You're not pushing and then coasting. You're still propelling, just with less effort.

For some people this is the ideal option. For me personally, it wasn't the right fit because of the added weight and what that means for transfers and loading a chair into my car. But I want to cover them because for someone without those specific constraints, they're worth knowing about.

Alber e-motion

The e-motion replaces your rear wheels with motorized hub wheels. Every push you make is amplified based on your input. Assistance level is customizable through the app. There's also a cruise mode that maintains speed without pushing, which is useful on longer flat stretches.

One feature I find especially worth mentioning: the rollback delay. On slopes, the wheels hold your position for up to 5 seconds so you can reposition your hands without rolling backward. For anyone with joint instability or fatigue, that's not a small thing.

Because each wheel operates independently and responds to each side individually, they can work for people with asymmetrical strength or limited grip on one side. This makes them more accessible than most rear-mounted options for certain presentations.

Good for people who want seamless, proportional assistance that responds to their own movement, and who can manage the added weight.

Quickie Xtender

The Xtender by Sunrise Medical works on the same push-to-assist principle. It senses force and adds motorized support proportionally. If you're already in a Quickie or Sunrise Medical chair, fitting and support may be more straightforward.

Worth asking your ATP to compare the Xtender and e-motion side by side, as feel and compatibility can differ.

Category Three: Front Attachments

Front attachments connect to the front of your wheelchair and pull you forward, turning your chair into a motorized three-wheeled vehicle. This is a fundamentally different experience than the other categories. You're not propelling. You're driving.

For people with very limited upper body capacity, or who want to cover serious distance, this category opens up something that the other options don't. The trade-off is size. These are larger devices. They require planning around car transport and storage. Most are not covered by insurance and are classified as recreational or personal transport devices.

Rio Mobility Firefly 2.5

The Firefly is the most accessible entry point into front attachments, both in terms of price and simplicity. After an initial installation on your chair that typically takes about an hour, it latches on and off in seconds. The center frame separates without tools for transport in a car or storage.

Top speed is 12 mph with five speed settings. Range is 12 to 15 miles per charge on a removable lithium-ion battery. It has separate forward and reverse throttles. It comes with a headlight.

It's designed primarily for urban environments and smoother surfaces, though it can handle light gravel and grass. It's manufactured in the US, which Rio Mobility notes as a point of pride for quality control.

Insurance rarely covers this one. But the price point is significantly lower than the Batec range, and for someone who wants to test front attachment freedom without the larger investment, it's a solid starting place.

Good for people who want front attachment capability at a lower price point, prioritize portability, and primarily navigate urban or suburban environments.

Batec Electric Handbikes

Batec is a Spanish company that makes what are widely considered the premium front-attachment handbikes for manual wheelchairs. They're certified medical devices under ISO 13485. In the US they're classified as personal transporters rather than medical devices for insurance purposes, so coverage varies and is worth verifying.

The Batec Easy-Fix anchoring system attaches in under three seconds, independently, without transfers. The company states compatibility with about 95% of manual wheelchairs.

They currently make six models for different needs and lifestyles:

The Batec Air is the lightest and most portable. It folds to hand-luggage size and has a top speed around 9 mph with about 14 miles of range. Designed for frequent travel and public transit use.

The Batec Mini 2 Plus is their most popular model and the one most often recommended as a starting point. It includes cruise control and traction control, folds for transport, and handles both city and mixed terrain.

The Batec Electric 2 Plus steps up in power with a wider tire and a 900 watt peak motor. Top speed around 15 mph. Built for users who need real all-day performance across varied terrain.

The Batec Rapid 2 Plus is for people who want speed and distance as the priority. Long routes. Open roads. Serious range.

The Batec Scrambler 2 Plus is the most powerful model in the range, designed specifically for off-road and challenging terrain. It has a 1,440 watt motor, a 19-inch knobby tire, and a claimed range of up to 34 miles. Top speed is around 12 mph.

The Batec Hybrid 2 combines manual pedaling with electric assist, for people who want to stay physically active with power backup available. Good option if exercise capacity is a goal alongside mobility support.

All Batec models are available in versions designed for quadriplegic and hemiplegic users, with specialized handlebar and control configurations for people with limited grip or one-sided function.

Good for people who want a high-quality, durable front attachment built for real daily use, longer distances, or terrain access that goes beyond what other options offer. This is a significant investment, but it's built to last.

Triride

Triride sits between the Firefly and the Batec in terms of power and capability. It's built for more demanding terrain and offers models with speeds up to 20 mph and ranges up to 30 miles. It's bulkier than the Batec but has a strong following among people who want serious outdoor and off-road capability. Worth looking at if distance and terrain are your top priorities and you want to compare against the Batec range.

So Where Do You Start?

If you're new to all of this, my honest recommendation is to start with an ATP evaluation if you haven't had one. An assistive technology professional can assess your chair, your body, your home, and your daily life and help you match the right device. They can also support insurance documentation if you're pursuing coverage.

If you're trying to figure out which category makes sense:

If you want to keep propelling with less effort: rear-mounted or power assist wheels.

If you want to preserve your chair's footprint and keep everything wireless: Empulse R90.

If you want to stop pushing and gain real range and terrain access: front attachments, starting with the Firefly if budget is the main constraint, or Batec if you want something designed for daily life.

The most important thing I want you to take from this post is that these options exist. Nobody handed me a list. Nobody told me at my evaluation. I found out because I kept looking.

You deserve to know what's available for your body before you spend years making do without it.
